Does Your Ankle Still Feel Weak Long After the Sprain Was Supposed to Heal?

Eric Finger physical therapist helps a man with Ankle Pain Physical Therapy in Austin, TX

Most people treat a sprained ankle with a few days of rest and ice, then go back to normal once the swelling drops. The problem is, less swelling does not mean the ankle has healed. An ankle that was never properly rehabilitated often stays weak and stiff, and it rolls again easily.

You might step carefully on uneven ground, feel your ankle give way without warning, or notice that it has never felt right since an old injury. That lingering instability is a real problem, and we can treat it.

Our approach to ankle pain physical therapy in Austin starts by finding what did not heal the first time. Because we come to you, we can watch how your ankle handles your own stairs, your driveway, and the surfaces you use every day. That tells us far more than a flat clinic floor.

Common Ankle and Foot Conditions We Treat

Ankle and foot pain covers many problems, and the right plan depends on which one you have. This list is not complete, but these are the issues we treat most often for our Austin patients:

  • Lateral ankle sprains and chronic ankle instability, the rolling, giving-way ankle that keeps coming back
  • Recurrent sprains that have left the joint weak and unreliable
  • Achilles tendinopathy, felt as pain and stiffness at the back of the heel
  • Plantar fasciitis and plantar heel pain, usually worst with your first steps in the morning
  • Peroneal tendon irritation along the outer ankle
  • Ankle impingement and limited dorsiflexion that pinch or block movement
  • Post-fracture and post-surgical stiffness, including recovery after a broken ankle

Whatever is causing your symptoms, your evaluation is there to find it and give you a clear path forward.

Why a Weak Ankle Is Never Just an Ankle Problem

Your ankle is the base of every step, so when it is unstable, the rest of your body adjusts to protect it. A guarded ankle changes how your foot hits the ground. That change travels up the leg and can lead to knee pain, shin splints, and irritation along the outside of the leg. It also throws off your balance, which raises the odds of another fall or roll. This is how one poorly healed sprain becomes a long history of them.

Being treated at home helps here. We watch you move on your own steps and thresholds, see how you push off when you walk, and retrain your balance on the surfaces you actually use. The stability you build then holds up in real life, not just on a clinic mat.

How We Treat Ankle Pain in Your Home

After a full evaluation, your therapist builds a plan around what is keeping your ankle from healing. It may include several proven methods:

  • Manual therapy and joint mobilization to restore the dorsiflexion a forward talus takes away
  • Balance and proprioception training to rebuild the position sense that keeps you from rolling
  • Therapeutic exercise to strengthen the calf, peroneals, and hip muscles that steady each step
  • Dry needling to calm irritated muscles and trigger points around the ankle and lower leg
  • Shockwave therapy to jump-start healing in stubborn tendon problems like Achilles and plantar issues
  • Blood flow restriction training to rebuild strength safely when the joint cannot handle heavy loads yet

If your ankle is still swollen and sore, we may add cold laser therapy to bring down the inflammation and speed the early healing. The plan always follows the same arc: first we calm the flare, then we restore the motion and strength you lost, and finally we retrain your balance so the ankle can protect itself.

In more than twenty years of practice, we have treated thousands of patients in hundreds of homes, gyms, and on sidelines around Austin. That experience taught us that no two unstable ankles are alike. A dancer, a trail runner, and someone who just wants to walk the dog without fear each need a different result. Our 20+ years let us build toward the specific life you want back, not a generic recovery.

If you are not certain which of these fits your situation, identifying it precisely is exactly what your evaluation is designed to do.

Studies Show Physical Therapy Works for Ankle Pain

The research is clear. The 2021 lateral ankle sprain clinical practice guidelines in the Journal of Orthopaedic and Sports Physical Therapy looked at both first-time sprains and chronic ankle instability. They recommend exercise, balance training, and manual therapy to restore function and, just as important, to prevent the repeat sprains that so often follow.

That prevention piece is the part most people miss. Good rehab is not only about recovering from the sprain you have now. It is about preventing the next one. This is why the balance and strength work matters as much as the early pain relief. At Move Empower Concierge Physical Therapy, this prevention-first approach guides every ankle plan we build.

I started working with Pawel at Move Empower Concierge Physical Therapy after breaking my ankle earlier this summer. I was pretty out of shape after barely moving while it healed. In just a few weeks, I’m noticeably stronger, my balance is much better, and I’m back to pretty much all my usual activities with almost no pain. What I really appreciate is that we’re working on my overall fitness too, with the goal of avoiding similar injuries in the future. We started very gently, and now I’m doing jumps and much more challenging exercises. Pawel knows how to push me just enough without overdoing it, and he keeps the sessions fun by changing things up. He’s always on time, easy to talk to, and clearly knows what he’s doing. Scheduling is flexible, and having him bring all the equipment to my house makes it so convenient. I’m really happy with how much better I feel overall. Working with Pawel has been one of the best decisions I’ve made for myself. Highly recommend!

I met Eric through clients of mine a few years ago and was impressed by the gentle and caring way he worked with them. I would often see Eric in their home during therapy sessions (another great thing about Eric - he comes to you!). He always brightened my clients’ day and was always very personable with me. When I broke my ankle running a few months ago, I contacted Eric about physical therapy. Not only did he help me recover faster from my injury, he helped me to understand the risks and the right way to get back into running. But we didn’t stop there. I’m back to running and feel better than ever, and I’ve continued to work with Eric on my fundamentals. He always wants to know how I’m doing, checks for any signs of stress injuries or potential issues, and he has taught me so much about running and physical conditioning. I’m happy to say I’m now running faster and stronger than ever, and with better technique than I’ve ever had. I can’t say enough good things about Eric. My daughter now works with him as well. I consider Eric an important ally in my and my family’s health. Not only is he an expert in what he does, he cares. Eric is an all around great guy.

Eric is an amazing physical therapist. I had shoulder surgery after 20 years of pain and needed PT, but kept putting it off. As a new business owner, getting to a PT clinic was challenging for me. But Eric comes to my office and works around my ever-changing schedule. The one-on-one attention has been wonderful. After working with Eric for several months, I am pain free for the first time that I can remember. And he has helped me with a lingering knee issue that wasn't improving on its own. I can't recommend Move Empower Concierge Physical Therapy enough.

After having surgery for a trimalleolar fracture of my ankle, a friend recommended that I contact Eric Finger of Move Empower to begin physical therapy, even before my surgeon suggested PT. And I'm grateful that I took her advice. Eric came to my house 2 weeks post-op (I was totally non-weight bearing for 6 weeks), evaluated not only the condition of my ankle but also my overall physical condition. Limited physical mobility affects other major areas of the body: back, hips, knees, etc. Eric took a whole body approach to my therapy and, as a result, 6 months later, not only have I regained 80 to 85% of my mobility (and have returned to golf and gardening, my 2 passions) but I have also strengthened and maintained my entire lower body: calves, hamstrings, knees, hips. This whole body approach is a huge difference maker. At 67 years old, I am now confident that I will return to my previous level of activity without any ongoing disability. I would not hesitate to recommend Eric and Move Empower PT. He definitely brings a superior approach to physical therapy. And that's what you deserve if you experience a serious injury.

I can't sing Eric's praises enough. In June 2021 I broke my ankle in 4 places. In just 3 months I was walking, chasing my kids around and exercising as I had before my accident. My doctor said I was in the 99th percentile of recovery. This is entirely due to working with Eric early (week two after surgery) and his fine attunement to my needs. As a busy mother of 2 small children, I don't have time to go to a PT facility. Eric came to my house twice a week and we started right away with rehab exercises. He even created programs I could do in hotels (as I travel for work) and exercises I could do while watching TV or hanging with my kids. PT doesn't have to feel like a full time job. His program works with your lifestyle so you can get back on your feet fast and get back to life after an injury. Invest in yourself and your life! Get this angel of a man to help you live your best life.

I hired Eric to work on my daughter’s ankle and have been very pleased. I have worked with physical therapists in the past and Eric is both knowledgeable and professional. Having the concierge service was so helpful with working around her school and sports schedule.

I had been avoiding seeking help for my heel pain due to preconceived notions that medical professionals were rushed and focused on treating symptoms rather than addressing root causes. Eric from Move Empower PT was neither of these things. I found him to be diligent, affable, and effective at diagnosing and treating the issues I was having. Aside from treating the heel pain directly, he identified weaknesses up the chain and prescribed exercises to address these. The exercises, while not overly time-consuming, made me stronger and more stable in my athletic endeavors. It's clear that he cares about his patients' progress and goals, given how he attentively listens and follows up. I would definitely recommend this provider to anyone experiencing stubborn or emerging pain!

I worked with Eric for a few months to help with an ankle sprain. He was awesome on all fronts: tooling, professionalism, flexibility in scheduling, diagnosing and iterating on PT, etc. 100% recommend him to all friends!

Ankle Pain FAQs

Mild sprains can heal on their own, but many do not fully recover, and that is the main reason ankles keep rolling. Physical therapy helps most when you have sprained the same ankle more than once, when it still feels unstable, or when your full motion and strength have not come back. That is when the cycle of reinjury sets in.

Repeated giving way usually means chronic ankle instability. It comes from stretched ligaments and lost position sense after earlier sprains that were never fully rehabbed. Both the strength and the balance side respond well to the right training, and that is what breaks the cycle.

This will depend primarily on the type of condition and the length of time the condition has been present. A chronic or hypersensitive condition can have more sensations than an acute injury.

Full rest is rarely the answer past the first day or two. Ankles regain their stability through movement and gradual loading, not through sitting still. Your therapist will guide how much to do and when to add more, so you stay in a safe and useful range.

Yes. An old injury that left you stiff, weak, or prone to rolling is very treatable. It is never too late to build the strength and balance that were missed the first time.
